VACATION TIME
• Eligible Employees: Full-time with three months service (minimum of 32 hrs/wk)
• Two weeks -- One through four years of service completed (3.077 hour per pay period)
• Three weeks -- Five through nine years of service completed (4.615 hours per pay period)
• Four weeks -- Ten-plus years of service completed (6.154 hours per pay period)
• Time accrues each pay period
• Part time (40 hours per pay period) accrue ½ fulltime benefits.
• Vacation leave will not be paid for less than one hour increments.
SICK TIME
• Eligible employees: Full-time employees who have completed three months of continuous employment (minimum of 32 hours/wk)
• Accumulate one 8-hr day per month of sick time
• Employee may accumulate up to 130 days (1040 hours)
• Part time employees accumulate ½ fulltime benefits.
HOLIDAY TIME
• Eligible Employees: Full-time and part time employees shall be eligible for holiday pay after 30 days of employment. On all holidays, all eligible employees will receive the holiday benefit on the official day of observance.
• New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas and a floating holiday.
BEREAVEMENT
• Up to three days for full-time employees (minimum of 32 hrs/wk).
TUITION REIMBURSEMENT
• Applications for tuition reimbursement are submitted to the Human Resources Department.
• Course must be job related and pre-approved by Human Resources Department.
• Full-time employees with one (1) year of service.
• $2000 maximum per calendar year – undergrad program.
• Must receive a grade of B or better if undergrad prog.
LIFE INSURANCE (Basic Term Insurance and AD&D)
• Eligible Employees: Full-time employees must complete one (1) month of continuous employment.
• One time base annual salary (non-exempt employees)
• Two times base annual salary (exempt employees)
• Benefit provided at no cost to employee
OPTIONAL LIFE INSURANCE
• Eligible Employees: Full-time employees (minimum of 32 hrs/wk).
• New employees will be subject to a one (1) month waiting period.

• Employee pays cost of $.21 per $1000 of coverage per month (Example: Salary of $20,000 provides $40,000 of coverage per year at $3.88 per pay period).
VOLUNTARY DEPENDENT GROUP TERM LIFE
• Eligible Employees: Full-time employees (minimum of 32 hrs/ week).
• New employees will be subject to a one (1) month waiting period.
• Life insurance for lawful spouse - $25,000; and
• Each unmarried eligible dependent children - $10,000
• Premium $4.52 bi-weekly.
VISION PLAN
• Eligible Employees: Full-time and part-time.
• New employees will be subject to a one (1) month waiting period.

• There is a prescription drug program with Caremark Scripts. It is totally separate from the Health Insurance Plan ($50 deductible ($100/family). When purchasing a 30-day supply of covered medications simply present your Caremark Script Card at a participating retail pharmacy. After your deductible is satisfied you will pay the following co-payment per prescription:

SECTION 125 PREMIUM PAYMENT PLAN
• Employee's premiums for employees’ medical, dental, vision and All State products are paid on a pre-taxis basis. This reduces the employee’s “real” cost.
LONG TERM DISABILITY
• Eligible Employees: Full-time (minimum of 32/hrs.wk) New employee must complete a 180-day waiting period. This is an employee paid benefit. The premiums are based on salary and age. This benefit pays 60% of basic monthly earnings to a maximum of $5000 after 180 days of total disability.
SHORT TERM DISABILITY
• Eligible Employees: Full-time employees (minimum of 32/hrs/wk)
• This is an employee paid benefit. The premiums are based on salary.
• Elimination Period is the length of time of continuous disability that must be satisfied before you are eligible to receive benefits. Elimination Period is 21 days.
• If you meet the definition of disability you would be eligible to receive a weekly benefit if you are disabled equal to 60% of your weekly earnings to a maximum of $1,000 per week.
401(k) RETIREMENT
• Retirement plan is provided in the form of a 401(k) retirement savings plan. Your participation in the Health Management Associates, Inc. Retirement Savings Plan is automatic. The 3rd plan administrator is Prudential. All full-time and part-time employees will automatically be enrolled in the HMA Retirement Savings Plan for a 4% Pre-Tax contribution. You have 45 days from your date of hire to decline your participation. Unless you direct otherwise, all contributions will be invested in the Guaranteed Income Fund.
• To make a different contribution election, or to decline enrollment, you must call: Prudential’s toll-free Answer Line #(1-877-778-2100) following your second payroll period. You may access their website www.prudential.com/online/retirement
You may choose to do the following:
1. Begin to contribute Pre-Tax contributions immediately, rather than wait for the automatic payroll deductions to begin after the 45-day waiting period.
2. Contribute more than the automatic 4% of your eligible pay (up to a maximum of 50%).
3. Contribute less than the automatic 4% (the minimum contribution is 1% of your pay).
4. Allocate your contributions to a fund or fund(s) other than the Guaranteed Fund.
5. Decline participation in the plan.
FITNESS PROGRAM
• Discount rates at All American Fitness Centers are available. Usually in October there is a representative here to conduct onsite enrollments. Call 737-8441 (Midwest City office) for enrollment information. (This includes, but not limited to Tanning, Aerobics, Kickboxing, Yoga and free personal training).
VOLUNTARY SUPPLEMENTAL BENEFITS
• All these benefits are paid in addition to any other benefit you may have and are paid to you unless you assign them to the hospital. All State’s benefits are designed to take care of the non-medical costs such as deductibles, co-payments, loss of income, travel for treatment and other family disruption costs. These optional plans will be available through HMA’s group discount and premiums also qualify for pre-tax savings.
UNUM PERSONAL ACCIDENT PROTECTION
• Eligible Employees: Part-time and Full-time employees who have completed a one (1) month waiting period.
• This benefit is designed to protect you and your family from an accident with 24-hour protection on or off the job. Benefits are paid for emergency room visits up to $150, hospital admission $750.00, hospital confinement $200.00 per day, follow-up doctor visits, crutches, physical therapy, transportation costs, etc…You receive up to $5,000 for broken bones, lacerations, ligament damage, etc. based on Unum’s schedule of benefits.
ALLSTATE SUPPLEMENTAL CANCER PROTECTION
• Eligible Employees: Part-time and Full-time employees who have completed a one (1) month waiting period. To assist you with the non-medical costs involved when treating a cancer illness, this plan pays you an initial diagnosis benefit of $5,000. Daily hospital confinement of $300, outpatient benefits up to $300 daily, the cost of transportation and lodging to special treatment centers is also covered. Surgical benefits up to $4.500 and many other benefits are available when treating the high cost of cancer. Also, there is a wellness benefit up to $100.00 annually for cancer screenings.
ALLSTATE CRITICAL ILLNESS INSURANCE
•Eligible Employees: Part-time and Full-time employee who have completed A one (1) month waiting period. Covers a severe, life threatening illness. Employees may choose amounts ranging from $10,000 to $30,000. This plan is voluntary and you can take the coverage with you if you leave the company.
FLEXIBLE SPENDING ACCOUNTS
• Eligible Employees: Full-time employees (minimum of 32 hrs/wk) who have completed a one (1) month waiting period.
• You will have the ability to obtain additional pre-tax advantages through your ability to set aside pre-tax dollars for Dependent Care as well as Unreimbursed Medical Expenses.
• The maximum is $5,000 for Dependent Care and the maximum for Unreimbursed Medical Expenses is $5,000.
• There is a 24-hr processing and your money can be direct deposited within 72 hours in your checking account.
